St. Mary Medical Center Foundation Now Accepting Grant Requests

St. Mary Medical Center Foundation announced Friday that is now accepting Letters of Intent (LOI) for the 2013 Community Grants funding cycle.

Eligibility Requirements

Successful applicants for funding must meet all of the criteria below:

Must be a 501(c) (3) organization or have a fiscal agent with the non-profit designation.

The program for which funds are being requested must already be in existence. We will not accept requests funding for a new program unless the proposal is from a multiple organization collaborative. Expansion and amelioration to existing programs are acceptable.

The program for which funds are being requested must support residents of St. Mary Medical Center’s service catchment area including the cities of Long Beach, Wilmington, and/or Signal Hill.

The program must also address a population who has one or more of the following:

  • Financial barriers to access (e.g., uninsured, underinsured)
  • Language/cultural barriers to access
  • Lack of transportation
  • Physical disability / lack of physical mobility
  • Mental disability
  • Social isolation (e.g., seniors living alone)

Fundable-Program Guidelines

Funds are being offered in two program areas of key concern to St. Mary:

  • Prevention and treatment of obesity and related chronic disorders:
  • Promotion of nutrition
  • Identification of chronic disease
  • Treatment for diabetes, high blood pressure, etc.
  • Promotion of mental wellness:
  • Prevention
  • Mental Health assessment
  • Treatment and Counseling

Must be a health-related project which integrates at least two of the following Dignity Health principles:

  • Focus on disproportionate unmet health-related needs
  • Emphasize primary prevention and address underlying causes of health problems
  • Contribute to a seamless continuum of care
  • Build community capacity
  • Emphasize collaborative governance

Collaboration among multiple service organizations and/or agencies is strongly encouraged. We prefer to fund organizations that show formal collaboration with other organizations including St. Mary Medical Center, and work with the population that St. Mary Medical Center serves.

To be considered for the Community Grants Program, your organization must first submit a letter of intent (LOI). Completed LOIs must be submitted by email to Janaya Nichols, Community Grants Representative, Janaya.Nichols005@dignityhealth.org

LOI Deadline: June 27, 2013 at 12pm

Late applications will not be accepted!

Letters of Intent will be reviewed by the Community Benefits Advisory Committee of St. Mary Medical Center. Organizations that advance to the next round will be invited to submit a full grant application on July 11, 2013.

Full Grant Proposal Deadline: August 5, 2013 at 5pm

Organizations will be notified of grant award in December 2013.

Grant Project Period: January 1, 2014 – December 31, 2014.

Grant Funding Amount: awards from $10,000 to $40,000 (with awards at the higher end favoring collaborative projects and/or partnership with St. Mary)

If awarded, a grant completion report will be required by March 31, 2015.

Grant Information Session

An information session on the Community Grants Program and the grant application will be held on Friday, June 7th at 10am in the Parr Room (2nd floor) of the Health Enhancement Center on the St. Mary Medical Center Campus at 1050 Linden Ave. Long Beach 90813. This information session is encouraged for interested applicants, but is not mandatory.
